Heart attack: the signs and the response
Central chest pain or pressure that may spread to the arms, neck, jaw or back, breathlessness, sweating, nausea and a sense of dread. Women, older people and people with diabetes often have vaguer symptoms: indigestion like discomfort, unusual tiredness, breathlessness without pain. Call 112 or 999 at once, say heart attack, and sit the person down in a half sitting position with knees bent and back supported, loosen tight clothing and keep them calm and still. If they are not allergic and can swallow, a single 300 mg aspirin chewed slowly is recommended by the Irish Heart Foundation; the ambulance crew will want to know it was given. Be ready to start CPR and send for the nearest defibrillator if they collapse.
Stroke: FAST
Face: has one side drooped, can they smile evenly? Arms: can they raise both and keep them up? Speech: is it slurred, jumbled or absent, can they repeat a simple sentence? Time: if any one sign is present call 112 immediately and note the time symptoms started, because the clot busting treatment is only given within a few hours of onset. Do not give food or drink, keep the person lying with head and shoulders slightly raised if conscious, or in the recovery position if unconscious and breathing, and reassure them; they can often hear even when they cannot answer.
What not to do
Do not drive the person to hospital yourself; the ambulance carries a defibrillator, oxygen and a direct line to the stroke or cardiac unit and can start treatment on the way. Do not let them walk it off or wait to see if it passes. Do not give aspirin for a suspected stroke, because a bleeding stroke would be made worse. And do not leave them alone.
